Below are samples of two different types of reading assessment reports.
The first report is tailored towards Early Readers and the second one is more appropriate for older students.
Consider the tests you are using in your outside testing and decide which format to use. Be sure to use a variety of informal measures
to gather information on the range of abilities your student brings to the task of reading.
Informal Assessment Sample (Early Reader)
Below you will find a file that contains a completed early reading assessment report as well as template.
You can use when reporting Dominie, DIBELS, CORE, etc.
(A starting template. Your exact assessments will depend on your progression of tests. You may cover all five components recommended by the National Reading Panel
or you may be assessing only a few targeted areas. Include in the introduction the rationale behind why you chose certain assessments.)
